Granbury CBOC serves the following counties: Eastland, Parker, Palo Pinto, Hood, Stephens and Callahan. A map is available under "View Map" on the right side of this page.
VA North Texas Health Care System has a Community Based Outpatient Clinic (CBOC) program to provide health maintenance and preventive health care within 30 miles or 30 minutes of a Veteran's home. CBOCs reduce travel time and shorten waiting times for Veterans who need outpatient care. Any Veteran in priority groups 1 - 8a, 8c is eligible to enroll.

If a CBOC provider determines the need for specialty care, a consult is submitted. Referral to a specialty clinic is coordinated with the nearest VA North Texas facility and the Veteran will be notified of appointment date and time.
Veterans interested in enrolling in the CBOC program may contact the Enrollment Coordinator at 1-800-924-8387 ext. 36759.
Once a Veteran is found to be eligible for enrollment, he/she will be added to the CBOC registry at the beginning of the next month. The contractor for that registry will send the Veteran a welcome letter which includes the phone number and address of the clinic closest to their home.

Veterans new to VA North Texas Health Care System will need to furnish a copy of DD Form 214 (Certificate of Release or Discharge from Active Duty). If a Veteran does not have a DD-214, one can obtained from the National Archives and Records Administration.
