Release of Information

The Release of Information department at all campuses (Dallas, Bonham & Ft. Worth) complies with applicable laws related to privacy practices and procedures when using or disclosing information. Our team is prepared to assist you with:

  • Accessing your medical records
  • Obtaining copies of your medical records
  • Requests to amend your medical records
  • Questions you may have about our privacy practices
  • Obtaining electronic records that may be available through My HealtheVet

How to Request Information

On plain paper, you may specify what information is requested. Your request must be signed and dated.  Please include your social security number so it will be easier to access your record.   It can then be mailed to one of our three campuses:

Electronic Access to Medical Records

The VA is slowly releasing medical information electronically through the use of the My HealtheVet web site.  Because these new features release more personal information to the veteran, additional security measures are required to protect each patient’s privacy.

In order to obtain this level of access, the veteran must
  • Be currently receiving care at a VA medical center.
  • Be a registered user of My HealtheVet.
  • View a video available on the My HealtheVet website or at VA libraries in Dallas and Bonham and in the Administrative Office at the Fort Worth OPC.
  • Complete a Release of Information form.
  • Present the form and a picture ID (veterans ID card or drivers license) to the Release of Information Desk at Dallas, Bonham or Fort Worth Outpatient Clinic.
Veterans who successfully complete this procedure will also, in the future, be able to check appointments, lab values, and other portions of the electronic health record when those capabilities are added.  These new features are not yet available.  They will be added a piece at a time and will be announced as they are released.Laa
