Anticoagulation (Protime) Clinic

The Anticoagulation Clinic is a required learning experience for the PGY1 pharmacy resident. The experience involves the provision of direct patient care for patients consulted to and followed by the Anticoagulation Clinic. The resident is scheduled in Anticoagulation Clinic one day a week for the duration of the residency. The resident is responsible for providing anticoagulation management under the supervision of a clinical anticoagulation pharmacist.  In addition, the resident will provide anticoagulation education to patients and their family.  Knowledge of warfarin, low molecular weight heparins, fondaparinux, dabigatran, thromboembolism, atrial fibrillation/flutter, heart valve replacements, vascular disease, and ischemic heart disease are important in this setting.
