Just three miles from Dallas VA Medical Center (VAMC), the Polk Street Annex will serve as a new model of care called PACT that will offer many benefits to our Veteran patients. PACT, or Patient Aligned Care Team, simply means patients will work together with a specific health care team to plan care and life-long health and wellness. Patients who move their primary care services to the Polk Street Annex will continue to receive prescriptions and specialty services at Dallas VAMC.

PACT will deliver excellence in every aspect of patient care, and focuses on:
o Partnership with Veterans
o Access to care using diverse methods
o Coordinated care among team members
o Team-based care with Veterans as the center for their PACT

Benefits of PACT:
• Assigned team: primary care provider, a registered nurse (RN care manager), licensed vocational nurse (LVN) and administrative clerk
• Better health care experience
• Better coordination of health care services
• More satisfaction

Coming Soon to Polk Street: Shuttle transportation to/from Dallas VAMC,
Mental Health, Nutrition and Social Work services.

Directions from Dallas VAMC:
From 4500 S. Lancaster, turn left from main entrance. Make a right onto Ledbetter Drive (TX-12-Loop W). Turn right on Ledbetter and drive approximately 3 miles to Brook Spring Drive (at Cliff View Crossing Shopping Center). Turn right on Brook Spring Drive. VA Polk Street clinic is on the right and may be accessed from parking lots.

Directions:
From Points North: Take I-35E South to US-67 South toward Cleburne. Take Polk Street exit, stay straight turn left onto South Polk Street. Clinic will be on the right.

From Points South: Take I-35E toward Dallas. Exit onto Loop 12 W/ E Ledbetter Drive. Turn left on Loop 12 and stay straight, passing South Polk Street. Turn right on Brook Spring Drive (at Cliff View Crossing Shopping Center). VA clinic will be on the right and is accessible from Brook Spring Drive.

Heading West on Ledbetter, make a right on Brook Spring Drive (at Cliff View Crossing Shopping Center). Proceed to VA clinic on right.

Heading East on Ledbetter, make a left on on Brook Spring Drive (at Cliff View Crossing Shopping Center). Proceed to VA clinic on right.

Public Transportation: Polk Street Annex is served by DART Bus Line and Light Rail.

To travel from Polk Street to Dallas VAMC: Take bus from stop (#28634) Ledbetter at Cedar Falls – East - Far Side on Bus Route 466 Buckner Station Eastbound to Ledbetter Station. Transfer at Ledbetter Station to Light Rail (Blue Line) Northbound Downtown Garland. Arrive at light rail stop (#23391) VA Medical Center station.
